Insurance Information

When you call our office to schedule your appointment, we will ask you first if you have any vision plan. This is an insurance plan that covers routine eye examinations and glasses or contact lenses. This is different from your health insurance (major medical). We will then ask you about your medical insurance. We will utilize your medical insurance information if your visit is related to an eye or medical condition that can affect your eyes. We will also be asking you about the Subscriber ID number, group number, name of the insured and date of birth of the insured. Please have this information available when you call for your appointment. We can then verify your insurance BEFORE you arrive for your visit.

We file most major medical insurance plans, including:
